Eyelid surgery is designed to remove the bags — wrinkles usually are dealt with peels and laser — you need to be seen, but the bags can be removed with minimal down time and almost no scarring.
Sometimes, filling around the wrinkles left can avoid reoperative surgery. This is tricky stuff. Only see a board certified Plastic Surgeon, or Oculoplastic surgeon about this
Most lower eyelid operations are aimed at removing any excess skin as well as treating any underlying fat bags. If you still have fat bags after surgery (give it at least 4-6 months for full healing to occur), then it would be prudent to return to your surgeon and discuss the situation. Or, you can consult with another plastic surgeon for a second opinion.
